We are excited and grateful that you are taking private lessons from us! Please make note of the following policies and procedures, and sign the form below. We will give you the top portion to take with you. (LESSONS WILL NOT BE GIVEN UNTIL BOTTOM PORTION IS SIGNED & RETURNED TO MUSICOPIA)
COMMUNICATION:
We handle all of the scheduling, billing and correspondence for the faculty. Contact by email (info@musicopia.com) or phone (708.749.7500) is best between the hours of 12-10pm Tuesday-Friday and 10am-8pm Saturday and Sunday. Calls/emails will be returned as soon as possible. Please note that calls/emails will not be returned on Mondays. In addition, please notify us of any family or school problems, as well as time consuming family activities which may be stressful to your child. Your child’s well being is our main concern.
PRACTICE COMMITTMENT:
Plan a regular time for DAILY practice. Ideally, you should practice 6 days a week; 30 minutes a day to make the most out of your private lessons for beginners. 45-60 minutes a day for advanced students.
LESSON SCHEDULING:
Please pick a day/time that is best for you for the duration of lessons booked. Make-up lessons are available (please see cancellation policy). Rescheduling can be difficult as slots may be filled directly before and after the ones you have chosen.
CANCELLATION POLICY:
YOU ARE RESPONSIBLE FOR ALL LESSONS YOU HAVE SCHEDULED!
When scheduling lesson times, make sure to pick a time that will work for ALL of your lessons. Alternate lesson dates and times are difficult to schedule and impedes the learning process. When a conflict occurs and it is prior to 24 hour notice, you may call to reschedule a lesson. Lessons that are cancelled/missed without a 24 hour advance notice will NOT be rescheduled. To better assure rescheduling, call early with any potential conflicts. Lessons may be rescheduled the same week OR may be doubled up on other/later lessons -----(If cancellation has occurred at least 24 hours in advance). If WE have to cancel a lesson, those lessons will be rescheduled that same week or doubled up at a later time. Lessons that are cancelled/missed without a 24-hour advance notice will NOT be refunded. Failure to notify us prior to 24 hours will result in full charge for that lesson.
LESSON TERMINATION POLICY:
YOU ARE RESPONSIBLE FOR ALL LESSONS YOU HAVE SCHEDULED!
When a student registers for lessons, we anticipate that this will be a long term relationship. However, if it is necessary to discontinue lessons, a FULL ONE (1) WEEK notice must be given to Musicopia so that we have the opportunity to find another student for that time slot. Lessons paid for and not used will be refunded by store credit only.
LESSON PAYMENTS:
Parents and/or students are expected to PREPAY for any and all lessons. Lesson days/times will not be held until payment is received. For your convenience, we accept VISA and MASTERCARD in addition to checks and cash. **Special payment arrangements may be available on an individual basis. ***A fee of $25.00 will be assessed for late payment.
SNOW EMERGENCY POLICY:
In the event of heavy snow, unless you hear from us, lessons are ON. We will call you if weather prohibits us from teaching lessons on a given day.

SIGNING THIS DOCUMENT INDICATES YOU UNDERSTAND, ARE RESPONSIBLE FOR, AND ARE IN COOPERATION WITH THE POLICIES DESCRIBED ABOVE. LESSONS WILL NOT BE GIVEN UNTIL THE FORM IS RECEIVED.
